I had a doctor's appointment to day at the Sunset Kaiser Clinic. It was simply a yearly wellness check. After seeing the doctor I needed to use the restroom. I've been going to the doctor totally en femme for about six or seven years now and I've been to the restroom there several times. As I refuse to use the men's room while not masquerading an average male, I've always used the women's room.
Today was the first time I encountered this sign outside the restroom.
l'm sure that they saw it as an invitation for trans-folk to use the restroom they felt comfortable in, but I saw it as fair warning to the cis population that trans-folk had a right to be in what ever restroom fit their gender.

I've seen lots of restrooms meant for one person of either gender.
Those are called "Family Restrooms"
This restroom was a standard multi stall women's room. Note the sign has only the woman icon on it. If it were as you suggested it would have this icon:

I haven't seen that at the Kaiser in Oakland. I hope it is coming soon, and not only there!
Monday was the first time I'd seen it
I've been going to that clinic for over 10 years and using that restroom the whole time since I started going en femme. I had to go get blood work after seeing the doctor, which meant going to the hospital next door. While there, I check and the restrooms there had the sign as well.
