Medical Dietary Accommodation Form
If your student has any medical dietary needs, please complete the Medical Dietary Accommodation Form and submit it to Food and Nutrition Services (FNS). This will ensure the cafeteria is aware of your student’s dietary needs and that the proper accommodations can be made.
