Gmail add-on stop working suddenly with permission error

Hi,
Our released Gmail add-on had been working on customer's Gmail account until last month . Then, for unknown reason, they experienced error 'Authorization is required to perform that action'
From my investigation, that error happen when the authentication on our system send callback request to the script. The problem is I could not reproduce the issue on my own Gmail account. I told customer to uninstall & reinstall and even changed the Gmail account, but the problem still persisted on their side.
Anyone has idea of what could be the error cause?
