Not registering dynamic tools

Is this a DOM scanner? - seems to be only registering the forms "toolname" attribute and not my script registered tools. Also a check on the input type for "Declarative Forms" seems to be failing hidden inputs and the submit button - I could be wrong but there is little information declaring which element is creating the warning flag.
