points straight up and kind of hurts to get it so it's at a 90 degree angle from my
body. I'm in my 20s and am still a virgin. I'm worried that this will interfere with
sex. Should this be a concern? Is there anything I can or should do medically so
that it can more easily be at a 90 degree angle?

The angle of the penis during an erection is different for all guys. Some are more angled than others, but it doesn't have an effect on the function of the penis.
That means you have a normal healthy functioning penis. Keep it that way, eat healthy (no junk food) and excerise often. You'll need it like that for the normal laying on top of her postion for sexual intercourse.
As you get older the penis straight up pointing should decrease.
