Fees
Two Separate Payments Are Required
- Check or money order, payable to the U.S. Department of State, for each application (see fees below).
- Check, money order, cash or credit card, payable to Moline Public Library, for each group of applicants. A $30 fee will be charged for returned checks.
All fees may be subject to change.
