Passport Service at the Moline Public Library

The Moline Public Library is an authorized United States Passport Acceptance Facility and accepts passport applications on behalf of the U. S. Department of State.

We only offer processing for first-time applicants for a passport book and/or passport card. Passport renewals are not accepted. 

We are not a Regional Passport Agency and do not provide physical passports on site.


All applicants and relevant parties must be present during the application process.

Both parents and children must be present for all minor applications (under age 16). If one parent cannot be present, you must submit a Parent Consent Form (DS-3053), completed with a notarized signature from the non-applying parent/guardian, and a copy of front and back of the non-applying parent's ID (it should be the same ID that they showed the notary).


Passport service is offered by appointment only. Passport Agents reserve the right to stop accepting applications prior to that time, if it means that the processing service will continue past closing.  Please allow adequate time for each application.

Each applicant must provide the following:

  • Current version of passport application form DS-11 (Completed and Unsigned)
    Complete form online ( and print OR print a blank form and fill out using black ink only.  Application forms are also available at the library. Fill out application completely, including social security number. Applications without SSN cannot be processed. Do not sign your applicationuntil asked to do so by a passport agent, who must administer an oath and witness your signature.
  • Evidence of citizenship
    Original or state certified copy of birth certificate (not a hospital certificate or photocopy), a previous passport, naturalization certificate, or a consular report of birth abroad.
  • Photocopy of evidence of citizenship
  • Proof of identity
    Current driver’s license, military ID card, government employee ID or past passport. For minors under the age of 16, the applying parent(s) must present ID in place of the applicant.
  • Photocopy of your identification document
    Copy must have both front and back of ID, preferably on a single page. Black and white copies are okay but the picture must be easily identified as the ID holder.
  • One recent 2”x2”color, full-face view photo
    Must have been taken within the last six months, with a white or off-white background, with normal street attire. No hats, headgear, uniforms or eyeglasses are permitted. Smiling or exagerrated facial expressions are also prohibited.
    The Moline Public Library does take passport photos on site.The photos can be taken at the time of your appointment. Photos cost $10.00 per applicant.

Two separate payments are required:

Check or money order, payable to the U.S. Department of State, for each application(see fees below).

Check, money order, cash or credit card, payable to Moline Public Library, for each group of applicants. A $30 fee will be charged for returned checks.

Expedited processing is available for an additional fee.  Please speak with a Passport Agent for more information.


Routine applications are processed within 4 to 6 weeks.  During peak times (March through September), processing times can increase to 8 weeks or more.

Expedited applications are processed within 2 to 3 weeks.
