自分の家の畳の上で死ぬのが最高だと言われる。しかし年取った患者さんの面倒が見
れる家族が殆どいないので畳の上で死ぬのは難しい現状にある。今訪問診療や訪問看
護体制はすごく充実してきているが、上述の理由で利用者は少ない。今私は84歳の女
性患者さんを受け持っている。認知症があったり腎炎があったりする患者さんだが、
食欲が全くなく、自分で食べることが出来ない。そこで胃ろう増設を家族に提案した
が断られ、鼻管栄養も拒否された。明日短時間だが外出して家に連れ帰ってみるとの
ことで、訪問診療や訪問看護があることをお話した。お嫁さんがどこにも勤めていな
くて家にいるとのことである。明日私は東京に行かなくてはならない。東京から帰っ
て来たら家族にその患者さんをどのように看護するか聞くことにしている。

visiting nurse has progressed sufficiently, but the user is few for the
reason of above.

chronic nephritis etc. She has not appetite and can't eat herself.
I advised her family to make an artificial nutrition fistula, but they
refused to make it and even also nasal tube nutrition. She will go home
tomorrow in short time. I told her family about the visiting doctor and
visiting nurse. Her daughter in law has no business and live in her house.
I must go to Tokyo tomorrow. I am going to ask to her family how to manage
