いろグラン甲州2024（まるき葡萄酒）Iro-grand Koshu 2024（Marqui Winery）
平日ど真ん中だったが、夕食のさんまに合わせて、甲州ワインを開けてみた。選んだのは先日訪れたまるき葡萄酒さんのもので、シュールリー製法によるものだそうだ。色調はごく淡いイエローで、少しばかり麦わら色がかったニュアンスがある。香りはシンプルだがクリーンなもので、ナシやライチなどの果実、小粒の白い花、それにほんのりとイースト香。飲んでみると、しっかりした伸びのある酸が果実味を支えて、なかなかバランスが良い。他品種を圧倒するようなキャラクターがあるわけではないのだが、控えめに和食に寄り添ってくれるという意味では貴重なポジションかもしれない。このシリーズは先日ベーリーAも飲んだばかりだが、どちらの銘柄も、肩ひじ張らない中にも抑えるべきところをきっちり抑えた立派な仕上がりだと感心した。長年のノウハウの賜物というべきものなんだろう。It was right in the middle of the week, but I decided to open a bottle of Koshu wine to go with grilled sanma (Pacific saury) for dinner.The one I picked was from Maruki Winery, which I visited recently, and it’s made using the sur lie method.The wine has a very pale yellow color with a slight straw hue. The aroma is simple yet clean, showing notes of pear and lychee, tiny white flowers, and a gentle touch of yeast. On the palate, a lively acidity supports the fruit, giving the wine a nice, well-balanced feel.It doesn’t try to outshine other varieties, but rather quietly complements Japanese dishes—which I find really charming. I also had the Muscat Bailey A from the same series not long ago, and both wines struck me as relaxed but well-crafted, with all the important details neatly in place. You can really sense the benefit of years of winemaking experience behind them.

ウインナ珈琲発祥の地～神保町ラドリオ
生クリームをたっぷり乗せたウインナ珈琲を日本で初めて出した店なのだそうだ。表通りから少し中に入ったところにあるが、それほど敷居が高い感じはしない。レトロな店内。たまたま入口間近の席に通されたので、入店する客をそれとなく観察できたが、この日は８割ぐらい外国人客だった。若い女学生と思しきバイトさんが頑張って英語で対応していた。ナポリタンは細麺で軽く焦げ目がつくぐらいよく炒めてある。タバスコが効いていて少しピリッとする。食事とセットで1300円で注文できる飲み物には、件のウインナ珈琲も含まれる。いや、それにしても、ものすごいボリュームのホイップクリームで、正直飲みずらい（笑）。仕事の合間のランチというよりは、オフの日にのんびりと午後のくつろいだひとときを過ごすのに使いたい、そんな印象の店だった。They say this is the first café in Japan to serve Viennese coffee topped generously with whipped cream.It’s tucked slightly away from the main street, but it doesn’t feel intimidating at all.The interior has a charming retro vibe. I happened to be seated near the entrance, which gave me a good view of the customers coming in—and about 80% of them were foreign visitors that day. A young part-timer, who looked like a college student, was doing her best to serve them in English. Their *Napolitan* spaghetti uses thin noodles, stir-fried well enough to get a nice touch of char. The Tabasco gives it a little spicy kick. The drink that comes with the meal set (¥1,300) includes their famous Viennese coffee. But wow—the mountain of whipped cream on top is something else! Honestly, it’s a bit hard to drink (laugh). It’s not really the kind of place you drop by for a quick lunch between meetings. It feels more like somewhere you’d want to visit on your day off—to take it slow and enjoy a relaxing afternoon.

神保町半チャン四天王のひとつ～中華そば伊峡（いきょう）
すずらん通りから入ってすぐのところにある昔ながらのラーメン店。創業は1966年、2019年に今の地に移転したのだとか。「ラーメン500円」という看板がいやでも目に付く。いつも混んでいるが、回転がよいので、並べばすぐに順番が回ってくる。ランチにあまり量を食べると、午後眠くなって仕方ないので、最近は量を控えめにしているが、この日は例外。なにせ「神保町半チャンラーメン四天王」または「御三家」のひとつというのだから、迷わず半チャンラーメンを選択した。ラーメンは非常にシンプルで、ラーメンというより中華そばという呼び方のほうが合いそうだ。チャーハンもさっぱりした見ためだが、食べてみると、きっちりと味付けがされていて、御飯もほどよくパラパラ。劇的に旨い！というものではないのだが、食べていて郷愁をそそられるような、ほっとする味だ。食べログのレビューを読んでいると、神保町は「半チャンラーメン」発祥の地らしく、この店はその中でも「四天王」とか「御三家」のひとつなのだそうだ。とはいえ、他の店は閉店していて、残すはここと「成光」さんのみなのだとか。ということで、遠からず、成光さんも訪問してみることにしたい。An old-school ramen shop located just a few steps off Suzuran Street.It was originally founded in 1966 and moved to its current location in 2019.The sign that reads “Ramen ¥500” is hard to miss. It’s always busy, but the turnover is quick, so your turn comes soon once you get in line.Since eating too much at lunch always makes me sleepy in the afternoon, I’ve been keeping my portions small lately. But this day was an exception. After all, this shop is said to be one of the “Four Kings” — or “Big Three,” depending on who you ask — of Han-Chan Ramen in Jimbocho. So I didn’t hesitate to order the classic Han-Chan Ramen set.The ramen itself is very simple—more like what you’d call chūka soba (old-style Chinese noodles) than modern ramen. The fried rice looks light at first glance, but once you taste it, you notice it’s seasoned just right, with each grain nicely separated. It’s not the kind of dish that makes you go “Wow, amazing!”—but it has that nostalgic, comforting flavor that warms your heart.According to reviews on Tabelog, Jimbocho is actually said to be the birthplace of Han-Chan Ramen, and this place is counted among its legendary “Four Kings” or “Big Three.” That said, most of the others have closed down, leaving only this shop and one called Seikou. I’ll definitely have to pay Seikou a visit before too long.

いろベーリーA2024（まるき葡萄酒）Iro-Berry A 2024（Maruqi Winery)
前回飲んだフジクレールのベリーAがかなり残念な味わいで、やはりマスカットベリーAに多くを期待するのは難しいのかなぁとやや悲観的になっていたのだが、今回開けたボトルはフジクレールのそれとは正反対の仕上がりで、実に好感の持てる味わいだった。中程度からやや淡めのルビーで、エッジは紫の色調。香りはトップノーズに硝煙や赤身肉。SO2がやや浮いている感じなところに、スクリューキャップとあって瓶内は還元状態だったと思われる。とはいえ、この手の香りは個人的に嫌いではないし、スワリングすると、みずみずしいイチゴやラズベリーなどの果実香も立ち上ってくる。味わいはミディアムからライトボディで、チャーミングな果実と柑橘を想起させる酸とが心地よく調和している。タンニンは穏やかだが、軽い中にもそれなりのコクもある。さすがにステーキなどに合わせるのはやや荷が思いが、逆にふだん白ワインを合わせるような場面でも幅広く飲むことができそうだ。総じて、化粧っ気の強かったフジッコとは正反対の、背伸びをしないスッピン的みずみずしさが好ましく、さすが最古のワイナリーだけのことはあると感心した。The last bottle of Fujiclair Berry A I had was rather disappointing, and I was beginning to think that perhaps it’s unrealistic to expect too much from Muscat Bailey A. However, the bottle I opened this time was the complete opposite of that Fujiclair one—beautifully made, and genuinely enjoyable.The wine shows a medium to slightly pale ruby hue with purplish edges. On the nose, there’s a hint of gunpowder and lean red meat. The SO₂ stands out a little, and given that it’s under a screw cap, the wine seems to have been in a somewhat reductive state. That said, I personally don’t mind this kind of aroma; once swirled, fresh strawberry and raspberry fruit notes begin to emerge.On the palate, it’s light to medium-bodied, with charming fruit and citrus-like acidity in pleasant harmony. The tannins are gentle, yet there’s a certain depth and substance beneath the lightness. It might be a bit too delicate to pair with a steak, but on the other hand, it would work well in many situations where one might normally opt for a white wine.Overall, it’s the polar opposite of Fujicco’s rather made-up style—fresh, natural, and unpretentious. I was impressed by its pure, bare-faced charm and couldn’t help but think, this is exactly what you’d expect from Japan’s oldest winery.

90歳の母が転倒して左指を骨折 My 90-Year-Old Mother Fell and Fractured Her Finger
90歳の母親が郊外でウォーキングをしていて階段を踏み恥ずして転倒、救急車で京王永山にある救急病院に運ばれた。診断結果は左指骨折。その他にも打撲であちこちに青アザができたほか、瞼や指の付け根の傷を縫合した。左手の指はポッキリと折れており、本来「手術適用」の初見とのことだが、90歳という年齢を考慮して、当面ギプスで固定して様子をみようということになった。また、転倒時に頭を打った可能性もあり、年配者の場合、後日血腫ができる可能性もあるとのこと。そんなわけで、入院にこそならなかったものの、しばらくの間、通院しなければならなくなった。といって、電車でとても一人で行かせるわけにはいかず、耳が遠くてコミュニケーションにも不自由するので、私が付き添わざるを得ない。件の病院は我が家から車で１時間半と遠いため、しばらくの間、毎週のように有休を取得して、母を病院に連れてねばならない。大変な事態だが、命にかかわるような大怪我でなかったのは不幸中の幸いか。My 90-year-old mother was walking in the suburbs when she missed a step on some stairs and fell, and she was taken by ambulance to an emergency hospital. As a result, she fractured a finger on her left hand, and she also sustained bruises all over her body. In addition, cuts on her eyelid and the base of her fingers had to be stitched. Since the hospital is about an hour and a half by car from my home, I’ll need to take some paid leave every week for a while to accompany her there. It’s a serious situation, but fortunately it wasn’t life-threatening.

フジクレール・マスカットベリーA樽熟成2022 Fujiclair Muscat Bailey A Barrel Aged 2022
中程度からやや濃いめのガーネットの色調で、エッジは少しオレンジが見える。香りは甘いバニラのような樽香のトップノーズ。スワリングするとカシスやラズベリーなどの果実にオレンジピールのようなニュアンスが出てくる。口に含むと、ややフォクシーな果実味と柑橘類のような「スッパイ」酸。タンニンは穏やかで、フィニッシュにはオークのフレーバーが酸と絡み、余韻は短め。樽の使い方などに技巧が感じられて、ある意味面白いワインなのだが、なんというか、女子高生のプリクラ写真のような不自然さを拭えず、あまり杯も進まない。このワイナリーは以前訪問したこともあって、特に甲州ワインには好感を持っているが、このボトルに関しては、リピートはないかなぁというのが正直な感想だ。Medium to moderately deep garnet in color, with a hint of orange at the rim. The nose opens with a sweet, vanilla-like oak aroma. Swirling brings out notes of cassis and raspberry, along with a nuance reminiscent of orange peel. On the palate, it shows slightly “foxy” fruit flavors and a citrus-like tart acidity. The tannins are gentle, and the finish—where the oak tones mingle with the acidity—is relatively short. There’s a sense of craftsmanship in the use of oak and overall winemaking, making it an interesting wine in a way, but somehow it feels a bit artificial—like a high school girl’s purikura photo—and I found it hard to reach for a second glass.I’ve visited this winery before and have a favorable impression of their Koshu wines in particular, but to be honest, this bottle isn’t one I’d be inclined to buy again.

ほどよく均整の取れたベリーA～マスカットベリーA2023（白百合醸造） Well-Balanced Berry A – Muscat Bailey A 2023 (Shirayuri Winery)
中程度からやや濃いルビーの色調で、エッジは紫色のグラデーション。香りはチェリーやラズベリーなどの果実にオレンジの皮、バニラなどのニュアンス。飲んでみると、心地よいクリーンな果実味とそれを受け止めるじんわりとした酸、穏やかなタンニン。香りにも味にも少しフォクシーなフレーバーがあるが、あまりそこを突き詰めると「角を矯めて牛を殺す」ことにもなりかねないので、このくらいでよいのかなぁとも思う。二日目になると化粧っ気が薄れて、オレンジピールっぽい果実味が全面に出てくるようになった。It shows a medium to slightly deep ruby hue with a purplish gradient at the rim. The nose offers nuances of cherries, raspberries, orange peel, and a touch of vanilla.On the palate, there is a pleasant, clean fruitiness supported by gentle acidity and soft tannins. Both the aroma and flavor carry a slight foxiness, but trying to suppress it too much might be like “killing the cow by straightening its horns.” At this level, it feels just right.By the second day, the make-up had worn off, and the orange-peel-like fruit character came forward more prominently.

Members have won numerous solo awards, including 1st place at the All Japan High School Competition, as well as top honors in Okayama Prefecture across high school, junior high, and elementary divisions.Currently, about 120 enthusiastic members—from kindergarteners to high school students—take part, including a Junior Class selected for the All Japan Children’s Chorus Festival. The choir continues to nurture young voices who love to sing, while achieving excellence on the national stage.

>そう考えると、当初のプランを変更して、「先に白内障の手術を受けて、黄斑上膜の症状が進むようなら改めて後日（数年後など）手術を受ける」というスケジュールに変更したほうがよいのではないかと思うようになった。それで、このことを３か月に１度の眼底検査の際にかかりつけの眼科医に相談してみた。ところが帰ってきたのは意外な答えだった。以下、眼科医と私のやりとり。「う～ん、といっても、左眼の白内障も今すぐ手術しなければならないほど悪くなってはいないんですよ。」「え？そうなんですか？でも、この２年ぐらいの間に、対向車や街路灯がにじむようになってしまって、今は夜間まったくクルマの運転ができないほどなんですけど…。」「白内障によるものかどうかは、片目ずつ別々に見てみるといいですよ。左眼の方が白内障が進んでいるので。左目が特に見えずらいというなら、白内障が原因ですが。」「いえ、そんなことはないです。右目でも見えずらいです。…ということは、今の目の見えずらさは白内障でなく、むしろ黄斑上膜が原因とか…？」「その可能性はありますね。実際、黄斑上膜については、左眼より右目の方が数値が悪いです。」「ということは、白内障の手術をしたからといって、それですぐに夜間運転をできるようになるということでないんですね（溜息）。‥といって、黄斑上膜の歪みは、年初から特に変わってはいなくて、なんかかんだでこの見え方にも慣れてしまったし、手術したからといって歪みが劇的に改善するわけではないんですよね？」「白内障についても、黄斑上膜についても、数か月に一度ずつ検査しながら、ギリギリまで様子を見るしかないですね。」…という感じで、結局なんの進展もないまま、ひきつづき、2～３か月に一回のペースで検査を続けることになりそうだ。まあ眼科に通うことについては、どのみち緑内障の目薬を処方してもらわねばならないので、特に問題はないのだが、当面、見え方の改善が期待できないのはシンドイところだ。まずはとりあえず老眼鏡の度を強くしてもらうしかないか…。When I thought about it, I began to wonder if it might be better to revise my original plan—having cataract surgery first, and then, if the macular epiretinal membrane were to worsen, undergoing another operation later (perhaps in a few years).I mentioned this idea to my regular ophthalmologist during my routine fundus examination, which I have every three months.But the response I got was rather unexpected.Here’s how the conversation went:Doctor: “Hmm… well, your left eye’s cataract hasn’t progressed to the point where surgery is urgently needed.”Me: “Oh, really? But over the past couple of years, I’ve started seeing glare from oncoming headlights and streetlights, and now I can’t drive at night at all.”Doctor: “To check whether that’s caused by cataracts, try looking with each eye separately. The cataract is more advanced in your left eye, so if the left one is particularly hard to see with, that’s likely the reason.”Me: “No, not really—it’s also hard to see with my right eye. So that means the blurriness in my right eye might not be from cataracts but from the macular epiretinal membrane?”Doctor: “That’s possible. In fact, your right eye shows worse readings for the epiretinal membrane than your left.”Me: “So even if I have cataract surgery, it doesn’t necessarily mean I’ll be able to drive at night again, right? (sigh) That said, the distortion from the epiretinal membrane hasn’t really changed since the beginning of the year, and I’ve somehow gotten used to this way of seeing things…”Doctor: “All we can do for now is keep monitoring both the cataract and the macular membrane every few months, until the last possible moment.”And so, with no real progress made, I’ll likely continue with checkups every two or three months.Going to the ophthalmologist isn’t a big deal in itself—I need my glaucoma eye drops anyway—but it’s tough knowing there’s little hope of any improvement in my vision for the time being.For now, I suppose the only thing I can do is get a stronger pair of reading glasses…

さて、そんなところに、というか若干日時が前後するのだが、カミサンが件の「黄斑上膜」の手術を受けることになった。というのも、私とこのような話をしていて、彼女自身も右目の見え方がおかしいということで眼科に行ってみたら、ビンゴだったのだ。カミサンの場合は右目だけだが、症状自体は私より進んでおり、例えば視力検査で、(例の)一部が欠けた〇の図形を見ても、どこが欠けているどころの話でなく、〇の中に黒いドットがいくつも重なって見えるのだそうだ。そうなるまでなぜ気づかなかったのだろとも思うが、そもそも片目ずつで見るという機会は一般的にそうないのかもしれない。なお、カミサンは酷い閉所恐怖症なので、手術は大学病院で全身麻酔で受けた。なので、手術の体験談自体はあまり参考にならないのだが、術後の様子に関しては、「視力自体は出るようになったが、見え方はあまり変わらない」と、あまり満足していない様子である。私の場合も、そもそも歪み自体まだそれほどでもないところに、手術をしてもそれがあまり回復しないと言われてしまうと、正直モチベーションはかなり下がる。というのも、この手術は入院や退院後の生活を含めて、おそらく1週間～10日程度は静養を余儀なくされる。それが両目となると、かなり仕事に差し支えるなあというのがひとつ。もうひとつは、両目同時に手術するわけにはいかないので、片目の手術をしたあと、それなりのスパンを開けなければならない。白内障の手術を同時にするとなると、片目の手術後からもう片方の目の手術が終わって視力が安定するまでの間、数か月間、とんでもない「ガチャ目」で過ごさねばならない。などなど、悩んでいるうちに季節は秋になってしまった。歪みを初めて認識してから9か月ほど経過したわけだが、病状が進行している様子はほとんどない（見え方は年初と変わらない）。一方で、老眼と白内障はますます進んでいる感じで、おかげで車の運転はいよいよおぼつかなくなってきた。そう考えると、当初のプランを変更して、「先に白内障の手術を受けて、黄斑上膜の症状が進むようなら改めて後日（数年後？）手術を受ける」という風に変更したほうがよいのではないかと思うようになった（もう一度つづく）Around that time—though the sequence of events is slightly out of order—my wife ended up having surgery for the very same epiretinal membrane.While we were talking about my own situation, she mentioned that her right eye also seemed off. She went to an ophthalmologist, and sure enough, she was diagnosed with the same condition.In her case, it was limited to her right eye, but the symptoms were more advanced than mine. For example, during a vision test using the familiar broken-circle chart, she didn’t just see a missing segment—she said multiple black dots appeared within the circle. I wondered why she hadn’t noticed earlier, but then again, most people rarely look through one eye at a time.Because she suffers from severe claustrophobia, she underwent the surgery under general anesthesia at a university hospital. For that reason, her surgical experience itself is not very informative for me. As for the results, she told me, “My eyesight has improved, but the way I see hasn’t really changed much.” She didn’t seem very satisfied.Hearing that dampened my motivation considerably. After all, even if my distortion is still mild, undergoing surgery now may not necessarily restore straight vision. On top of that, I would likely need to take three to five days off work for hospitalization and recovery, which could seriously interfere with my job—especially if both eyes eventually require treatment.Another issue is that both eyes cannot be operated on simultaneously. After one eye is done, a waiting period is needed before the other can be treated. If cataract surgery is performed at the same time, that means I would have to endure several months with highly uneven vision until the second eye is finished and stabilized.And so, while I kept weighing my options, autumn arrived. It’s now been about nine months since I first noticed the distortion, yet my condition has barely changed—my vision looks the same as it did at the start of the year. Meanwhile, presbyopia and cataracts are steadily advancing, and driving has become increasingly unreliable.Given all this, I’ve begun to think I should change my original plan. Perhaps it would be better to undergo cataract surgery first and then, only if the epiretinal membrane progresses, consider surgery for it later—maybe a few years down the line.(To be continued once more)

ぶらり東京散歩37（番外編）〜巾着田の彼岸花2025/10/02 Tokyo Strolll37 Speciial Edition The Red Spider Lilies of Kinchakuda
10月２日（木）、たまたま休暇をとれたので、埼玉県の巾着田まで彼岸花を見に行ってきた。2022年以来だから３年ぶりだ。ちなみに３年前に行ったのは９月24日だった。酷暑の影響で、開花時期がかなり後ろにずれたのだろう。上流沿いは花の色が褪せ始めていたが、下流沿いはまさにピークの様相で、おそらく今週末ぐらいまでが見ごろだろうと思う。なお、今年はミラーレス一眼を持参せず、すべての写真をエクスペリアで撮影したのだが、ミラーレスで撮影した数年前の写真と比べると、日向や日陰、逆光など光の条件によって花びらの発色が不安定になる。街中で撮る分には十分すぎると思っていたXperiaだが、こうして本格的に撮ろうというときにはやはり一眼を持ち出すべきだったと少し後悔。↓こちらが３年前の記事https://plaza.rakuten.co.jp/szwine/diary/202209240000/October 2 (Thursday)Since I happened to have the day off, I went to Kinchakuda in Saitama to see the red spider lilies.It was my first visit in three years, the last time being in 2022. Along the upper stream the colors had already begun to fade and it felt like the peak was over, but along the lower stream the flowers were in full bloom. I think the best viewing will probably last until around this weekend. For reference, three years ago I went on September 24. The extremely hot summer this year must have delayed the blooming considerably.This time I didn’t bring my mirrorless camera—every photo was taken with my Xperia. 今年の初めあたりから急に物が歪んで見え始めたことについて。眼科医は否定するのだが、きっかけは緑内障の目薬を変えたことではないかと思っている。昨年の秋ぐらいから、アレルギーなのか、とにかく目がかゆくて、常時真っ赤に血しているような状態が続いていた。緑内障治療に点眼していた「アイラミド」を中断したら症状が改善したので、眼科医に相談して、目薬を「ミケルナ」に変えてみた。ところが、そこから１か月もしないうちに、前述の通り、視界にわずかではあるが歪みが生じているにに気づいた。https://www.kozuki-eyeclinic.com/preretinal_membrane.phpこちらのサイトに格子状のセルフチェック画面がある。片目ずつこのチェック画面を見ると、ラインがまっすぐに見えないのだ！左眼で見ると、水平の線の真ん中が少し凹んで見える。反対に右目で見ると、同じ水平のラインが左の橋に向かって下がっているように見える。垂直の線は左眼で見ると下半分が左に流れて見え、右目では右側に流れて見える。そして、これが実に不思議なのだが、両眼で見ると、きちんとまっすぐに見えるのだ。脳が補正しているのか、それとも左右で打ち消しあっているのか…。実生活に不自由はしていないものの、片目ずつ瞑ると、エクセルのスプレッドシートや道路のラインが歪んでいて、実に気持ち悪いし、さらに症状が進むようなら日常生活にも差し支える。この「黄斑上膜」、薬などで治すことはできず、治療法は外科手術しかないとのこと。白内障と違って、目の奥の網膜を弄る手術なので、素人目には結構大変な手術に感じられるが、手術機器の発達により、以前より身近にはなったようだ。通常は術後数日間入院するが、病院によっては、日帰り手術を行うところもあるとのこと。なお、この手術をすると、必ずといってよいほど白内障が進行してしまうので、年配者は通常、白内障の手術も一緒に行うのだそうだ。眼に針状の器具を挿して手術をするというのは怖いが、手術自体は1時間程度で済むらしい。それならいっそ、このところ悩まされていた白内障と併せて手術を受けてしまうのがよいのではないか。そう思って、いつもの眼科医に相談したら、隔週でその筋の専門医が来ているので、その先生に相談してほしいとのこと。日を改めて訪問し、症状を説明したところ、その医師もう～んと腕組みして悩んでいる様子。というのも、私の場合、右0.9～1.0、左0.8前後と、結構視力が出ており、画像診断上も、それほど進行している所見ではないらしいのだ。黄斑上膜においては、視力が0.6～0.7以下になってきたら手術を検討するのがセオリーのようで、歪んで見えるのがどうしても気になるというなら手術しますけどね…と、あまり積極的に手術を薦めてくる様子はなかった。あとで調べたところ、実のところ、黄斑上膜の手術は、「進行を止める」という効果はあっても、歪み自体は元に戻らないことが多いらしい。まあ言われてみると、視界の歪み自体は、前述の通り両目で見る分には気にならないので、当面様子をみてからでもいいのかなという気になった。それで、概ね仕事が一段落する来春ぐらいに手術を受ける予定にしておいて、それまで経過観察しましょうか、ということになったのだった。（つづく）Since the beginning of this year, I suddenly began noticing that objects appeared distorted.Although my ophthalmologist denies it, I suspect the trigger may have been switching my glaucoma medication.Around last autumn, my eyes became constantly itchy and bloodshot, perhaps due to allergies. When I stopped using the glaucoma drops “Ilamid,” which I had been taking, the symptoms improved. After consulting my ophthalmologist, I switched to a different medication, “Mikelna.”However, within less than a month, I realized that, as mentioned earlier, my vision had developed a slight distortion.When I look with my left eye, the middle of a horizontal line appears slightly indented. With my right eye, the same line seems to slope downward toward the left edge. Strangely, when I use both eyes together, the line looks perfectly straight. Perhaps my brain is compensating, or maybe the distortions are canceling each other out.In daily life this hasn’t caused major inconvenience, but when I close one eye at a time, Excel spreadsheets or road markings appear warped, which feels unsettling. If the condition worsens, it could interfere with everyday activities.This epiretinal membrane (ERM) cannot be treated with medication—the only option is surgery.Unlike cataracts, this procedure involves operating directly on the retina, which sounds daunting for a layperson. However, thanks to advances in surgical equipment, it has become much more common than it used to be. Normally, several days of hospitalization are required, but some hospitals even perform the operation on an outpatient basis.It is also said that this surgery almost inevitably accelerates cataract progression, so in older patients cataract surgery is usually done at the same time.The thought of inserting needle-like instruments into my eye is frightening, but the procedure itself apparently takes only about an hour. That made me wonder whether it would be best to undergo the surgery now, combining it with cataract surgery, which has also been troubling me recently.When I consulted my regular ophthalmologist, he suggested I speak with a specialist who visits the clinic every other week.I went back on another day and explained my symptoms. The specialist listened carefully, arms folded, clearly deliberating. In my case, my vision is still fairly good—around 0.9–1.0 in the right eye and about 0.8 in the left. The general guideline is to consider surgery for ERM when vision falls to 0.6–0.7 or below. He added that if I found the distortion bothersome, surgery was possible, but he did not strongly recommend it.Indeed, as I noted earlier, the distortion isn’t noticeable when both eyes are open, so I began to think it might be fine to wait a little longer. In the end, we agreed that I would continue monitoring the condition and, once my work has settled down next spring, plan to undergo surgery around that time.(To be continued)
