At Geriatric Health and Care Management, we are nurse/care managers and trusted advisers who assist families and senior adults in navigating the complex healthcare system. We offer a comprehensive assessment of medical conditions and care needs that assist families in decision-making. Our service model is individualized and allows the senior adult the highest level of independence and autonomy while maintaining safety and dignity. Our patients usually have one or more chronic conditions which often include dementia. Our care coordination services allow seniors to remain independent for as long as possible in their own homes and if desired in independent or assisted living. We work closely with adult children and help them to have less stress in caring for their parents/family members. Our advocacy services foster coordination of care with caregivers, primary care and specialty physicians, home care services, rehabilitation center therapists, and social workers. We also provide guidance related to end-of-life care decisions including hospice care. We can provide nursing assessments and transitional care post-hospital or rehab stay, ongoing monitoring of chronic conditions, medication management, accompany to physicians appointment, education and care plan development for caregiving services, assistance with long term care benefits and veterans benefits. We can make recommendations related to choosing assisted living communities and assist with transitions to those communities.
