Requesting Accommodations for College Board Tests

The information from this application form will be utilized to electronically file your request to the College Board by Langley High School’s Services for Students with Disabilities (SSD) Coordinator. While the SSD Coordinator is available to help facilitate this process, students/parents are ultimately responsible for completing the packet in a timely manner. It will require a minimum of up to 3 weeksto electronically submit your application so make sure you are aware of testing deadlines and plan accordingly.  Please know that though Langley forwards information to the appropriate organization and the school has no role in the decision process.  

Eligibility for an IEP/ 504 Plan does not constitute an automatic approval of College Board accommodations.

Each of these steps must be completed before the application can be processed.

Step 1. Please review basic criteria for College Board eligibility.  

Step 2.Complete this R equest College Board Accommodations document and email to  the appropriate SSD coordinator, including all required documentation  (partially completed forms will be returned to the applicant). 

Step 3.Print, complete, sign and attach appropriate consent form:

Step 4.Provide all recent documentation requested by the College Board 

  • A copy of the most recent IEP or 504 Plan
  • A copy of the most recent Physician diagnosis on letterheadand recent psychological/educational testing (within the last three years and in PDF or Word format)

Step 5.  Returnall materials (cover sheet, consent form & documentation) to appropriate SSD Coordinator below:

Requesting Accommodations for ACT Tests

The information from this application form will be utilized to electronically file your request to the ACT by Langley High School’s Services for Students with Disabilities (SSD) Coordinator. While the SSD Coordinator is available to help facilitate this process, students/parents are ultimately responsible for completing the packet in a timely manner.
