Start of Year School Forms

Step 1: Make Sure You Have a SIS Parent Account

  • SIS Parent Accountwill give parents access to ParentVUE , Parent Digital Consent System (PDC)and Schoology .  
  • You should have received an activation letter from the school with a code after you enrolled. If you have received a letter, proceed with activating your account.  If you do not have a letter, or need additional help with your SIS account, please contact Shelly Koshuta , our registrar.
  • Having an account is crucial to staying connected to your student's information. This is the preferred method for parents to update student information such as: phone numbers, emergency contacts, health conditions, and immunization dates and opt outs.
  • Before school begins parents should log in to both ParentVUE and the Parent Digital Consent System (PDC) to review and update all necessary information.  

Step 2: What Forms Need to be Completed?

During the initial weeks of the school year or when a new student enrolls, crucial information is shared. Various forms are available to caregivers via the Parent Digital Consent System and SIS ParentVUE , requiring completion. Below you can find more information to help guide you as needed. 

A SIS ParentVUEaccount is required to complete many of these online forms and to access the Parent Digital Consent System (PDC).

Emergency Care Card: Updated Annually - SIS ParentVue

Your child's Emergency Care Card must be reviewed for accuracy and updated every year if changes are needed. 

Please review and update the Emergency Care Card Information in  ParentVue . Make sure to fill out the “Current Health Conditions” and/or “Current Medical Care Details."

Health Information Form: Updated Annually - SIS ParentVUE

Health Information forms are required for each student. (Newly registered students do not need to complete IF completed at registration).

Online Verification Update Packet: Updated Annually-SIS ParentVUE

Online Verification Update Packet (OVU)  can be accessed from ParentVUE accounts. OVU allows parents to conveniently update phone numbers, emergency contacts, health conditions, and immunization dates for students. It is accessible through ParentVUE accounts.

Returning families or whenever there is a change with existing accounts, please log in to review and update your information before the start of the school year so that we have your family's most up-to-date information. 

SR&R Signature Sheet: Required for Each Student Annually-SIS ParentVUE

The Student Rights and Responsibilities (SR&R) handbook is located online in SIS. Parents and guardians are responsible for acknowledging they have received and reviewed the SR&R and should be prompted to do so when they log into their SIS Parent Account. Students will be reviewing the SR&R during the month of September.

Parents/guardians will digitally sign the SR&R in  SIS: ParentVUE. Every parent/guardian with a ParentVUE account, regardless of relationship to the student, will see the SR&R upon login to ParentVUE until the SR&R acknowledgment has been completed.  If a parent chooses, he/she/they may “skip” the SR&R acknowledgment by clicking on “Remind me later!”. The next time the parent/guardian logs in the SR&R acknowledgment will appear again.

The parent/guardian will NOT be required to click on the pdf document link to download/view the SR&R packet before the parent/guardian will be able to digitally sign and acknowledge the SR&R. The name in the digital signature box must match the parent first and last name in SIS exactly before the signature will be accepted.  Confirm the exact match with the "Welcome Message" in the top right of the screen where the parent's name is printed on the screen. 

CLICK on "SEND FORM." Changes will be updated electronically in our Student Information System (SIS). You do not need to return the paper copy.

⏩ Please submit the SR&R signature sheet online using  ParentVUE . If you need assistance accessing the SR&R Resources or the Signature Sheet please contact the school for guidance. 

Parent/Guardian Digital Consent: Updated Annually-PDC

Teachers will use a variety of digital resources to help students learn during the current school year. These digital resources have been checked and approved by Fairfax County Public Schools. 

Explanation of Form: As part of our commitment to protect student data, FCPS is required to secure parental permission for student use of software/applications for which FCPS does not have a confidentiality agreement. For ease of access, an online process has been developed to collect this information digitally and provide school-based reporting.

You can find more information about FCPS’ commitment to student privacy at  Digital Privacy in FCPS  and a list of all approved digital tools is viewable online in the FCPS Digital Ecosystem Library  which has links to the vendors’ privacy notices and other useful information.

This permission will expire at the end of this school year and can be revoked at any time by contacting the school.  If you have questions about any of the digital resources listed below, please contact your child’s school.

Parent Retest Permission form for State Assessments: Updated Annually - PDC

The Virginia Board of Education allows elementary and middle school students who fail their spring Standards of Learning (SOL) tests by a narrow margin to retake the test before the end of the school year. Retesting is available when a student fails their first SOL attempt with a score of 375-399. (A score of 400 is considered passing for SOL tests.) Note that retests are not offered for the integrated reading and writing (IRW) component of the grade 5 and grade 8 SOL reading tests.  A decision not  to allow retesting will not impact your student’s course grade or academic record.

⏩ Parent/guardian permission is required for the school to retest any student in grades 3-8. If you believe that your student would benefit from a second opportunity to demonstrate their knowledge and skills on the spring SOL test, please submit your decision using the Parent Digital Consent System (PDC) .

⏩ If you have any concern that your child may have scoliosis, make sure that your child is screened by a health care provider. The school Public Health Nurse (PHN), Kaitlin Marcelo , may provide screening for individual students at the request of the parent or guardian.

Family Life Education (FLE) Opt-Out Forms: Updated Annually - PDC

Marshall Road Elementary provides a comprehensive, sequential Family Life Education (FLE) program for students in grades K-6. Instruction is seen as a partnership among parents and guardians, the school, and the community in supporting the learning essential to the development of strong families, positive relationships, and a healthy community.

Parents/guardians may choose to opt their child out of all or part of the Family Life Education program. Students who are opted-out are provided with age-appropriate, nonpunitive alternative health instruction, and every effort is made to foster respect for family choices.

Health Room Forms and Medication Authorizations: As Needed

As needed, over-the-counter medications may be transported to school (directly to the health room) by a parent or designated adult. The medication must be in the original unopened container and accompanied by an appropriate Medication Authorization Form . They can be dropped off at our Open House as well as any time during the year.

Homeless Student Referral Form: As Needed

Confidential form if needed for families experiencing homelessness.

MySchool Bucks-Online Payment for Lunch and Other School Services

MySchoolBucks is an online payment service that provides a quick and easy way to pay for school-related purchases. You will be able to pay for a variety of school fees, including field trips, meals, and using credit/debit cards or electronic checks.

Lightspeed Parent Reports on Child's Device Activity

FCPS has tools to help parents know more about their child’s activity on any FCPS device. These tools are provided at no cost to families by Lightspeed, the school division’s internet content filtering service.

Parents may sign up for a  weekly Parent Report , which will provide a list of the top sites your child visited. The report also offers parents the ability to sign up for free access to the Internet Use Parent Portal. Through the portal, parents may pause their child’s access to the internet on their FCPS device during non-school hours.

Here Comes the Bus app is a school bus tracing app that can simplify morning and afternoon routines for parents and students by giving them real-time bus location and text or email notification alerts, helping them to get to the bus stop on time. News You Choose: Roadrunner Report

News you Choose is a subscription-based, email communication tool used to share communications such as newsletters, events, news updates and other topics. Parents can subscribe to newsletters as well as unsubscribe when you no longer want to receive newsletters from us at Marshall Road. Marshall Road sends out a weekly school newsletter every Monday afternoon called the Roadunner Rundown.   You can sign up for News You Choose and manage your subscriptions, and view our view the current and past Roadrunner Rundown newsletters on our website.

Talking Points for Families

Talking Points is a valuable tool to improve communication between families and teachers. Important messages from the school will be sent directly to your phone as text messages and will be translated into your home language. To respond to these messages, simply download the free Talking Points app and sign up using the phone number you provided during school registration. Please note that once you download the app, you will no longer receive text messages,  so it’s a good idea to allow notifications from the app to stay updated. 

Step 1: Downloadthe Talking Points for Families app for iPhone or Android

Step 3: Send messages in your home language that will be translated to English for your child’s teachers!

If you decide later to stop using the app, you must LOG OUT of the app to revert back to receiving text messages. Deleting the app from your phone DOES NOT restore text communication.
