Required and Optional Forms

Required Forms for All Students:

  1. Emergency Care /Health Form

    All students must have accurate emergency care information each school year. Parent/Legal Guardians must review and update your information via OVU Online Verification/Update   thru your SIS ParentVue Account. Log into your  SIS ParentVUE account

  2. Digital Resource Consent - This parent consent allows your child to use FCPS approved digital tools that require parental consent and can be completed in the Parent Digital Consent System .
  3. Parent Retest Permission Form for State Assessments in the Parent Digital Consent System .
  4. After School Program Registration Form (ALL students must complete, regardless of intent to participate) in the Parent Digital Consent System .
  5. Option to go Paperless: In  SIS ParentVUE - Under Report Card Tabs choose "Go Paperless"
  6. Intent to Ride School Bus - In  SIS ParentVUE

Opt-Out Forms available through the Digital Resource Consent System.

The following opt-out forms can be completed as needed by going to the Digital Resource Consent System and logging in with your SIS ParentVue credentials:
