Maximum weight is 50lbs for Males and 40lbs for Female Dogs 

Males need to have 2 testicles

Female Dogs can Not be in heat. 

If your dog is 6 months or older, you are required to have a current Heartworm test and be on prevention. 

Proof of Rabies, Distemper, Deworming, Fecal  is required for all pets.

If your pet has never been to a vet before we need to screen your pet to make sure they are a candidate for surgery and get them up to date with their vaccines and dewormer
