Based in Hartford, Conn., Chelsea Place Center, LLC is a health care center. It provides dementia care program, rehab services, sub-acute specialty programs and dietary/social services, to name a few. The health care center offers an array of services, such as ambulation training, cognitive retraining, adaptive equipment education and home discharge preparation, to name a few. Additionally, Chelsea Place Center, LLC provides patient/family education, home discharge preparation and activities of daily living re-training, among others. It also conducts recreational programs, including arts/craft, sports/exercise and pet therapy.
