Porthaven Post Acute in Portland, OR provides compassionate 24/7 skilled nursing and rehabilitation in a warm, modern setting. Located in the historic Cully neighborhood, the facility offers comfortable private and semi‑private rooms and encourages family involvement with flexible visiting hours. Our interdisciplinary team designs personalized care plans for post‑surgical recovery, joint replacement rehab, stroke and cardiac care, pulmonary support, vascular wound management, tracheostomy care, cancer recovery, bariatric care, and pain management. We provide physical, occupational, and speech therapy six to seven days a week to help residents regain strength, balance, and confidence. Residents enjoy restaurant‑style dining, transportation, monthly activities, social services, and case management, along with amenities such as Wi‑Fi, cable, activity rooms, walking paths, a chapel, fitness spaces, and cozy lounges. We accept Medicare, Medicaid, VA, PPO/HMO, and private pay.
