Medication & Treatment Management: Overseeing complex prescription regimens, managing side effects, and preventing dangerous drug interactions. Proactive Health Monitoring: Conducting routine clinical assessments (vital signs, heart/lung checks, cognitive shifts) to catch health changes early. Care Coordination: Communicating directly with specialists, hospitals, therapists, and pharmacies to keep everyone on the same page. Crisis Prevention & Navigation: Managing post-hospitalization transitions, rehabilitation plans, and urgent health concerns to avoid unnecessary ER visits by catching early signs in symptoms before they turn into major concerns.
