Checkup Medical provides many of the same services you receive at a doctor's office in the comfort, safety and convenience of your home, apartment, assisted living facility - any place you call home. We care for individuals age 18 and older, and specialize in keeping complex, frail and elderly patients out of the hospital so they can remain independent at home. In-home medical services we provide include, but are not limited to: patient-centered primary medical care; complex, chronic care management; prescription refills and management; diagnostic testing, including X-ray's and EKG's; blood work and lab testing; flu and other immunizations; injections; wound care consultations and management; diabetic screenings and management; acute illness and minor urgent care; transitional care visits following a hospital stay; and care coordination with other physicians, home health agencies; medical equipment companies, social service providers and other healthcare sources as needed to ensure consistency among all contributors.
