Pinnacle provides supportive, compassionate, skilled home health services for adults and seniors who are recovering after a hospital or facility stay or need additional attention to remain safely at home and avoid unnecessary hospitalization. Under the watchful eye of our integrative team of providers, our goal is to help each patient regain and maintain independence.
We believe in honoring God in all we do and provide individualized care plans for each patient. We are a State Licensed and Certified Home Health and Hospice Agency by CMS. Home Health Care services include Skilled Nursing, Therapy (PT, OT, ST), Social Services (Licensed Social Worker), Certified Home Health Aide, Personal Assistance Services, Post Hospital Care, Disease Management, Specialized Therapies and Programs. We are currently accepting PHC (through Medicaid) clients. Our Hospice and Palliative Services include Skilled Nursing, Social Worker, Chaplain, Certified Home Health Aide and Medical Director.
