Behavioral Health Consultant- LPC or LCSW

TCC Behavioral Health Consultants (BHC’s) are a part of an integrated team within our pediatric primary care clinic. They provide brief, evidenced based assessment and intervention services with the goal of supporting TCC patients and families with a wide variety of behavioral health needs.

Taking a generalist approach, the BHC’s work alongside our medical staff, as they provide behavioral health consultation support via warm handoffs, assessment/screening, and curbside consultation with the physicians. Additionally, our BHC’s see patients with non-acute mental health concerns for short term follow up appointments that typically last 30 minutes. Our behavioral health team embraces the primary care behavioral health model, which focuses on enhancing the primary care team’s ability to provide excellent treatment and management of care to TCC patients and their families. Our BHC’s work within a fast-paced setting and often thrive on being flexible, adaptable, approachable, and team-focused.

Clinical Care/Case Manager

The Clinical Care Manager is responsible for managing, overseeing, and guiding care coordination staff, ensuring safe, patient-centered care team services for patients and their families. In addition to management, they will manage and coordinate the course of treatments with physicians and clinical staff ensuring quality patient outcomes are achieved within established time frames. Duties will include conducting initial and ongoing assessments, determining, and managing outcomes, ensuring continuity of care, and utilization of resources. Manages a team of coordinators in the delivery of programs meeting Patient-Centered Primary Care Home (PCPCH) standards.

CMA or LPN- Portland

The Certified Medical Assistant/ LPN provides clinical & administrative support to clinicians & other healthcare team members, offering various healthcare services to patients. As appropriate to the position’s scope of authorized duties, the CMA/ LPN is required to have competent observational, clinical, critical thinking, communication, problem-solving & decision-making skills to meet patient needs & contribute to a positive care experience & work environment. The position works under the clinical supervision of the clinician for patient care activities, & the general direction of the designated manager/supervisor.

Patient Access Specialist

The Patient Access Specialist is often patients’ first contact when they visit one of our sites or facilities. The position is responsible for answering and distributing all incoming telephone calls to the correct destination. Greet and check in patients, verify insurance information, and collect copayments and outstanding account balances. Schedule patient appointments according to clinic protocol. Acts as a liaison in reporting any patient suggestions to management.
