Licensed Clinical Social Worker or Licensed Professional Counselor

TCC Behavioral Health Consultants (BHC’s) are a part of an integrated team within our pediatric primary care clinic. They provide brief, evidenced based assessment and intervention services with the goal of supporting TCC patients and families with a wide variety of behavioral health needs.

Care Coordinator

The Care Coordinator is responsible for providing social support while helping patients and their families to navigate and access community services and other resources, adopt healthy behaviors, and facilitate continuity of care by providing client follow-up. Duties will include using reports, health information technology, and registries to identify patient and population gaps in care, collaborate with the health care team/provider to confirm needs, communicate with patients about needed care, and follow up to ensure patients receive care. You will be responsible for conducting these activities directly and collaborating with others. This includes developing workflows and ensuring that population management activities across the organization are aligned and coordinated.

CMA or LPN- Portland

The Certified Medical Assistant/ LPN provides clinical & administrative support to clinicians & other healthcare team members, offering various healthcare services to patients. As appropriate to the position’s scope of authorized duties, the CMA/ LPN is required to have competent observational, clinical, critical thinking, communication, problem-solving & decision-making skills to meet patient needs & contribute to a positive care experience & work environment. The position works under the clinical supervision of the clinician for patient care activities, & the general direction of the designated manager/supervisor.

Advice Nurse

The Advice RN provides clinical & administrative support to clinicians & other healthcare team members to offer various healthcare services to patients as well as being responsible for overseeing the telephonic nursing advice services, providing leadership to the advice nursing team, and ensuring the delivery of high-quality care in accordance with pediatric best practices.

Laboratory Technician-Portland

As a Laboratory Technician, you are responsible for receiving, testing, analyzing, recording and reporting results of their tests.

Patient Access Specialist-Portland

Works in partnership with the clinical and administrative teams to successfully provide exceptional care to the pediatric population we serve. Daily responsibilities include assisting in coordinating activities of the front office-end operations department, including scheduling, registration, new patient coordination, and medical records.
