Transcript and Records Requests

Current Seniors:Complete (1) the form for current seniors and (2) the consent form .

Return all forms to our Transcript Assistant, Ms. McCabe.  These can be emailed to @email  ( must be sent from your fcpsschools.net student account ) or dropped off in room L107 (look for the tray below the green sign with the pink arrows).

For underclassmen students:

Unofficial transcripts can now be accessed in SIS.  Please follow this link for instructions:  https://www.fcps.edu/preview-link/node/48097/ad96bd7d-20dd-49ba-81a5-9baf528235f1

For official transcripts and other records, parents must complete (1) the form for underclassmen students and (2) the consent form . 

Return all forms along with a copy of your photo ID to our Transcript Assistant, Ms. McCabe, at @email .

If you attended or graduated within the last 5 years:

1. Complete and sign - Past Attendees/Graduate Request Form
2. Complete and sign - Consent Form
3. Copy of your photo ID
4. Make your payment via MySchoolBucks (see payment link below).  Most requests are $5 each.  Once all forms are turned in and payment is confirmed, your request will be processed within 48 hours.
