FCPS Required Forms for All Students
Emergency Care Form
- Parents can update in SIS ParentVUE or fill out the hard copy going home in students' Thursday folders
- If updating online, please return the hard copy marked "updated online"
Health Information
- Complete and return the hard copy going home in students' Thursday folders
- OR complete in SIS Parent VUE
- Click on DOCUMENTS tab
- This will walk you through how to update the "Health Information Form" which is an annual first week of school requirement
Student Rights & Responsibilities
Students will review the Student's Rights and Responsibilities (SR&R) in the second week of school. The SR&R Booklet can also be found on the FCPS website
. Parents should sign the SR&R form in SIS ParentVue
.
