Required and Optional Forms

Required Forms for All Students:

1. Emergency Care /Health Form
   • All students must have accurate emergency care information each school year. Parent/Legal Guardians must review and update your information via OVU  Online Verification/Update   through your SIS ParentVue Account.
     • Log into your  SIS ParentVUE account
2. Digital Resource Consent  - This parent consent allows your child to use FCPS approved digital tools that require parental consent.

      4.  Intent to Ride School Bus - In  SIS ParentVUE

Optional Opt-Out Forms available through Parent Digital Consent System as needed:
