Forms to Start the School Year
Information and links for forms students and parents need to submit to start the school year
Start of Year Forms
Parents and Students need to turn in and submit the following forms at the start the school year:
- Emergency Care Card: This was mailed home and should be received by August 10.
- If all of the information is correct , sign the form and return it at Tiger Fest.
- If corrections/updates are needed , the Enrolling Parent should submit them through SIS ParentVUE using the Online Verification (OVU) process by clicking “ Online Packets” in the upper right corner after logging on to ParentVUE no later than August 12. Submitting the OVU ensures we have access to the most up-to-date data prior to the school year starting and is the preferred method of submission.
- Science Safety Contract: Review the safety contract points with your student. Student and Parent/Guardian sign electronically using this Twain MS: Science Student Safety Agreement Google Form .
- Consent Forms in Parent Digital Consent System: Use your ParentVue account to log into the consent site
. You will receive a code in your email attached to the account to complete the login process. Please review the 3 required digital consent forms. You may select for all forms to be translated through this system.
- Digital Resource Consent 2025-26: Resources used at the middle school level that require parent/guardian consent can be found on this Digital Tools List . Twain does not use all of them, however they will be available as an instructional tool for teachers when needed. You may consent to all digital tools or go through the list consenting to individual tools. Directions for accessing the site can be found here.
- Parent Retest Permission form for State Assessments: Complete this form to let Twain know if you would like your student retested on SOLs if they qualify. You may consent to all retests or for specific subjects.
- MS After School: Review and acknowledge the After School Program Acknowledgement of Access, Policy & Risk. This must be complete to attend the after-school program. You will also be asked to identify if your student has a parent/guardian actively connected to the Army. You do not need to be Army-connected to participate in the After-School Program.
