AltaMed PACE Can Help Keep Seniors Healthy and Independent at HomeThe Program of All-Inclusive Care for the Elderly, or PACE, was established by Medicare to help independent seniors with complex medical needs. The program helps participants avoid nursing homes and instead, receive the care and services necessary to help them stay safe, comfortable, and healthy living in their own home.PACE offers care coordination, transportation, social services, and meals, all in a positive, comfortable environment. Its health care that puts the focus on care, connecting seniors with peers in their community as well as the PACE staff who are committed to treating participants with dignity, respect, and compassion.
Wells House of California is a medical and rehabilitation facility for people post-hospitalization. We have 24/7 staff to treat medical problems and a physical therapy team to rehabilitate our residents so they can return to their previous way of life. Facility staff include nurses, physicians, respiratory therapists, dietitians, social workers, and case managers. Our goal is to treat medical problems such as wounds or IV therapy. Some people only need physical therapy. Wells House is a sub-acute facility. We are able to care for people on ventilators or tracheostomy. Our goal is to wean residents off the vent and eventually de-cannulate (remove) a patients tracheostomy, if of course the patient is a candidate for trials.

We offer a comprehensive array of health and other supportive services to older adults and/or adult with chronic health condition. Our Goal is to delay and prevent institutionalization or other maore expensive care setting by improving and preserving each individual's physical, mental and emotional well-being. As each individual's need varies, our services are provided on an individualized basis.
