Required and Optional Forms 2025-26

Parents, 

Here are a few Back-to-School tasks:   

  1. Sign-up for the Jackson MS News You Choose

  2. Activate your ParentVUE account if you haven't already.

    If you are attending Tiger Time, on August 14 from 1-4 p.m., you can get your activation key and activate your account at that time.

    If you are not attending Tiger Time, you may request a ParentVUE Activation Key .

  3. Update forms in  SIS ParentVUE , must be updated yearly. These forms can be completed and submitted online through SIS ParentVUE and the new OVU.
  4. Review all the Tech Tools Available in FCPS .
  5. Engage in the Jackson MS Community through volunteering, joining the  PTA , working with teachers, etc.

Forms available in SIS ParentVUE :

  1. Log into your  SIS ParentVUE account .
  2. Emergency Care Form . All students must have accurate emergency care information each school year. Parent/Legal Guardians update this information through WeCare@School via their SIS ParentVue account.
  3. Health Information Form.

Forms available through Parent Digital Consent :

  • Digital Resource Consent - This parent consent allows your child to use FCPS approved digital tools that require parental consent.

  • SEL Screener Opt-Out
  • Consent for Release of Student Records in Support of Postsecondary Applications
  • Family Life Education Opt-Out
  • SOL Retest Permission

  • Objection to Release of Information to Outside Organizations
  • Objection to Release of Directory Information to the Public
