薬の副作用をよく学ばなければ危険. It is dangerous not to lean the side effect delicately.
今東京で行われた高齢者医療に関する講習会から帰ってきたところである。その研修会には全国から丁度100人の医者が集まった。高齢者医療について学ぶことは大変有益だった。この研修を通じて多くの役立つ情報を得ることができたが、そのうちの一つ、薬の副作用について紹介する。高齢者はいろいろな病気を持っているので、あっちこっちの医師を受診して一杯薬を処方されている患者さんがいる。ある薬は認知症のような副作用を引き起こすことがあるので、それを見つけて鑑別するのは難しいが、そのようなケースではその薬を除かなけれなならない。我々はもっと薬についてきめ細かく学んでいかなければならないと思った。I came home just now from the workshop for the old person's medicine atTokyo. Just 100 doctors from all area of Japan were gathered for thisworkshop. It was very useful to learn the old person's medicine. I learnedmany useful things in this workshop, I introduce one of it, the case of theside effect of drugs.As old patients have many disease ,some have been prescribed too muchbecause they have visited several doctors. Some drugs cause the badcondition as like dementia, we must avoid the drugs in such cases eventhough it is difficult to find differential diagnosis. We must study moredelicately about medical drugs.

認知症にならないようにするにはどうしたらよいか？The prevention of dementia
今朝、約2年間病んでいた義弟が明け方亡くなったとの連絡を受けた。心より冥福を祈る。今日は東京の秋葉原で、高齢者医療についての講習を受けた。多くの素晴らしい講義に感銘を受けた。例えば認知症の予防法や高齢者が沢山の薬を処方されて新たな病気の原因を作っているなどである。今回は認知症の予防についての講義を紹介したいと思う。今日本では400万人の認知症と400万人の認知症予備軍がいて、65歳以上の老人の１５％以上は認知症もしくはそれに近い人達である。年々その患者さんは増えている。認知症の原因はアルツハイマー病や脳血管障害その他があるが、その予防についての明白なエビデンスはまだない。しかし今日の講師は最も重要な予防策は運動と年取ってからもぼんやり過ごさないで何かに挑戦していくことだと信じていると述べた。私もそう思う。どんなに年とっても運動を心掛け、あらゆることに挑戦していこうと思う。I have a call from my wife that my brother in law died in this early morning. He was suffered during about 2 years . I pray for the souls of him. I have a lesson for the old person`s medicine at Akihabara today. I was impressed many nice lessons , for instance the prevention for dementia or too much medication for the old person may be the cause of another disease etc. I introduce this time about the prevention of dementia. We Japanese have about 4 hundreds dementia patients and 4 hundreds quasi-dementia patients, they are over 15% of over 65 year old. They are increasing year by year.The causes of dementia are Alzheimer disease , cerebrovascular disease or others, there is still no clear evidence for prevention of dementia, but the speaker of today said he believe the most important prevention way for dementia is exercise and challenging to something not living vaguely in the older life. I think so too, I will exercise and challenge to everything even how many getting old.

高齢者医療を学ぶために今東京に来ています I am now in Tokyo to lean old person’s medicine.
明日からの高齢者医療の研修会に参加するために今私は東京に来ている。明日の土曜日は、8時45分から老年期の諸問題、認知症、リハビリテーション、口腔ケア、栄養、薬物療法などについて5時５５分まで学ぶ。日曜日は8時半から5時35分まで、各症例の実例について実習を行う。この研修が高齢者医療の現場で役立つように身に染みて取り組もうと思う。I am now in Tokyo to attend the medical institute for elderly person from tomorrow.We learn from 8.45 AM tomorrow Saturday, containing many old person’s problems, dementia , rehabilitation , oral care, care of the decline , nutrition or drug therapy till 5.55 PM.On the Sunday , we must practice from 8.30 AM to 5.35 PM about the example of each case. I will intend to study hard to avail this workshop for the medicine of advanced years person.

日本シリーズ2勝2敗 Japan Baseball Series , each team 2 wined 2 lost.
日本ハムファイターズが今日3対１で勝った。広島東洋カープとファイターズは全く互角である。今年のシリーズは全く胸をわくわくさせる対戦である。どちらが優勝しても満足である。Fighters wined a game by a score 3 to 1 today. Carp and fighters are allsquare. It is very exciting series this year. We are satisfied if which teamwill be champion.

経験か若さか？Experience or youth?
山梨大第一内科のゴルフコンペに本日参加した。私は数週間前にゴルフ用具一切を失ってしまったので昨日それを買わなければならなかった。その新しいクラブに慣れなくて、スムーズにプレイすることができなかった。結果は17人中13位だった。私はゴルフの経験は一番長いと思うが、多くの若者たちに負けてしまった。正しく練習していなければ経験は役に立たないのだと思う。若者に勝ちたいと思ったら、もっと正しく熱心に練習しなければだめだと思った。I attended the golf competition of 1st internal medicine of Yamanashiuniversity today.I had to buy the golf clubs yesterday as I lost the all material of golfseveral weeks ago.I didn’t become used to use the new clubs, and didn’t play smoothly. I was13th in 17 attendants.I have the longest experience for golf, but I was beaten by many youngerplayers.The career is not so useful if a person haven’t exercise rightly. I mustexercise more rightly and earnestly if I want to defeat the youngerattendants. I thought so.

別離の悲しみを軽くする。Mitigating the pain of parting．
ここ数か月で多くの友人や知人と別離しなければならなかった。また長い間に多くの患者さんの臨終にも立ち会ってきた。彼らはどこに行ってしまうのであろうか？ 私たちは善行を積んできた人は天国に、悪行を重ねて来た人は地獄に落ちると聞かされてきた。しかしほとんどの人は人生の間にいいことも悪いこともしてきた中間の人たちである。これらの人達はどちらかに行くのであろうか？ たぶん彼らは、天国でも地獄でもない普通の世界に行くのだと思う。つまり我々の殆ど全ては普通の世界に行くのである。我々は天国や地獄がどんなであるかを教えられてきたが普通の国がどんなであるかは教わらなかった。それは目の前の現実の世界と変わらないのだと思う。家族や友人や知人と別離すると悲しみに暮れるが、もし彼らは別の普通の世界に住んでいると考えるなら、その悲しみは和らぐのではなかろうか。I had to part from many my friends or acquaintances in those several months.And I have also witnessed with the death of many patients during long time .Where do they go? We are heard that if he or she had done a benevolence, onewould go to the heaven, and if one had done badness, one would go to hell.Almost people had done a benevolence and badness in a life, they areintermediate. Where do they go, heaven or hell? Provably they will goaverage world not heaven ,not hell. In other words, we almost all will go tothe common world. We are taught how the world of heaven or hell, but nottaught how the common world. I guess that world is not difference as realworld in front of us. We feel pain when we part from family , friend oracquaintance , but if we think they live another common world, the pain willmitigate.

安倍首相は解散権を行使すべきではない！Prime minister Abe shouldn't dissolve a parliament !
日本では総理大臣が解散権を持っている。彼の望みが叶いそうなチャンス或いは不信任された時にはその権利を行使する。今は与党に対する支持率が高いので憲法改正できるチャンスと考えて国会を解散しようとしている。イギリスの議会の任期は５年で解散は許されていない。彼らは解散や選挙のことを心配しないで任期中は熱心に着実に仕事することが出来る。日本では国会議員は常に解散を恐れてびくびくしながらの仕事で本気で仕事ができない。解散はただ首相を利するだけで国民や国会議員にとっては何もいいことのない最悪のことである。諸悪の根源だと思う。法律で禁止すべきだ。Prime minister have the authority to dissolve a parliament in Japan.He dares to use the authority when he think the chance that his desire would go well or he is drove by non-confidence.He is going to dissolve a parliament now because he is thinking it is chance to able to change the constitution as the rate of support for the ruling party is high. An administration of the Parliament of UK is 5 years and the dissolution is not permitted. They work hard eagerly during the administration without anxiety for the dissolution or election.As for Japan , the members of the Diet are always are afraid of dissolution and can’t work seriously.The dissolution is the worst system for nation and the Diet member but only prime minister.I think the dissolution is the root of all evil. It should be prohibited by the law.

ショックで立ち上がれそうもなかった Shock !! I couldn't restart.
10月23日の山梨大第一内科のゴルフコンペに出るつもりだったので、先週の土曜日にゴルフ練習場に行き、きょうも又行った。車のトランクを開けてびっくりしてショックを受けてしまった。そこにあるはずのゴルフバックがなかったのだ。恐らく先週の土曜日練習場の駐車場に置き忘れてきたのだと思う。練習場や警察署に聞いてみたが何の情報も得られなかった。ゴルフクラブがなければゴルフコンペにも出られないし、練習もできない。ショックでどうしょうもないくらい落ち込んでしまった。家内にも中々言えなかったが、かなり経ってからおずおずと打ち明けた。相当怒られると覚悟していたが、意外にも怒らないで、大きな交通事故を起こしたのに比べればそれほど大きなことではないと励ましてくれた。恐らくゴルフバックは戻ってこないと思うが、家内の言葉で少し救われた。気を取り戻さなくてはと思う。As I am going to attend the golf competition of the first internal medicine of Yamanashi medical school at October 23 , I went to the golf driving range exercise center of last Saturday, and I went to there today too. I was surprised and shocked when I opened the trunk of car, there was no golf bag. Probably I left my golf bag in the parking area of the exercise center last Saturday.I asked the exercise center or police center, but there was no information.I can't attend the golf competition if I have no golf clubs, also I can'texercise before the competition. I was shocked and severely depressed. Icouldn't easily say to my wife about my mistake , but I said to her at lastafter a long time. I prepared for the anger of my wife, but she didn’t getangry surprisingly and consoled me saying as it was not so big accidents compared to severe traffic accident. Probably the bag will not be recovered, but I was relieved slightly by the words of my wife. I must compose myself.

ラミレス新監督は成功したが、高橋監督や、金本監督は最低だった。New manager Ramirez is successful, while Takahashi and Kanemoto are failure.
セリーグの3人の新監督のうち、DeNAのラミレス新監督は素晴らしい成果を上げたが、巨人の高橋、阪神の金本監督は選手時代はスーパースターだったのに、監督としては最低だった。巨人や阪神は能力の高い選手を沢山抱えているので、優勝争いをするだろうと期待されていた。しかし彼らは各選手から最高のものを引き出すことができなかった。ベイスターズはいい選手がそれほどいなかったのでそれほど期待されていなかった。しかしラミレス監督は、各選手の一番いいものを引き出した。試合中の高橋や金本のダッグアウトでの表情はあまり良くなかった。ポーカーフェイスだったり、苦虫を噛み潰したような顔をしていた。それでは選手達を鼓舞することは出来なかった。ラミレスは明るく振る舞い各選手を信頼していた。試合に負け後でもいつも明日がある。明日頑張ろうと言ってきた。高橋監督や金本監督は選手を信頼しているとはとても思えなかった。来シーズンには生まれ変わって指揮を取ってもらいたいと思う。もっと選手を信頼し、自分の選手時代の栄光は忘れるべきである。監督は自分のためではなく、チームのため、各選手のために働くべきである。Among the 3 new managers of the cetral leag, Mr. Ramirez was succesful ,but Mr. Takahashi and Kanemoto were at the minimum, while they had been a superstar as the player.Giants and Tigers were expected as a champion because they have many highlevel players. But those two new manager didn’t bring out the best in eachplayers.Bay stars was not so expected because they have not so many nice players.Manager Ramirez bought out the best from each players.The faces of Takahasi and Kanemoto in the dugout during the game were notnice, they were sometime porker face or making a sour face. It didn'tinspire the players.Mr. Ramirez was cheerful and trusted the each players. He said every timeswe can fight tomorrow even though after being beaten. It was not seen thatmanager Takahasi or Kanemoto trust the players.I expect them to be reborn next season. They should trust the players more,they should throw-out the past glory as a player. They should work for teamand for each players not for themselves.

これから痒み対策を立てる Preparation to protect the itching.
庭のコスモス今日は寒かった。暑い日は過ぎ去ってしまった。これから痒みと闘っていかなければならない。私の肌は乾燥肌で弱いので毎年寒くなると激しい痒みに悩まされる。ローションや軟膏、抗アレルギー剤などを準備して肌を守る対策を立てていかなければならない。It was cold today. Hot days have gone. I must fight the itching from nowonward. I am suffered by severe itching every cold season as my skin is dryand weak.I must prepare to protect my skin using the lotion, ointment or antiallergic drugs from now.

女性リーダーに期待するが男性にも頑張ってもらいたい。I expect woman leader but also man leader too.
今朝ラジオで今は多くの国で女性が大統領や首相になっており、英国やドイツ、韓国、台湾などいっぱいある。そしてアメリカも11月の大統領選で女性が大統領に選ばれる可能性が高いという話を聞いた。日本の首都東京都の知事も女性である。これからはもっと増えて女性の時代になっていくのではないかと思う。女性は優しい心を持っているので、もし彼女らが世界を収めていけば戦争は減っていくと思う。女性リーダーに期待するが、同時に強く、正しく優しい男性リーダーをも期待する。I listened that many country are administered now by woman president orprime minister, for instance England , Germany , South Korea, Taiwan etc.and the USA also will be governed by woman president probably after thepresidential election on November on radio this morning. Governor of Tokyois also female. It will be woman age in future. Woman have gentle mind. Ifthey rule the world the war will decrease. I expect woman reader but alsostrong, right and gentle man leader too.

歯痛や歯槽膿漏に対する最高の薬、The most effective ointment for the toothache or pyorrhea.

I was also suggesed to pull out a tooth as Ihave pyorrhea by a dentist about one month ago. I have toothache whenI bite and also have bleeding and pain of gums when I brush my teeth.I tried Hinopolon oral ointment and was surprised that it cured all mysymptoms. When I visited another dentist after using the ointment, I asked if I need to extract the teeth.He said there is no need to pull out if I have no pain or bleeding. 辻井伸行さんのピアノ演奏に驚嘆 Piano concert by Nobuyuki Tsujii was very wonderful.
ニューヨークでの辻井伸行さんの野外ピアノコンサートをたった今テレビで観た。それはとても素晴らしいものだった。一緒に演奏してくれた楽団の人達や、聴きに来たニューヨークの方々はあまりに優れた演奏に驚嘆していた。かれは生まれつき全く目が見えない。それなのになんであんなに上手に弾けるのか分からない。私は目が見えるのだからもっと真剣にピアノを学ばなくてはいけないと思った。I watched an outdoor piano concert by Nobuyuki Tsujii in New York on TVjust now. It was very wonderful. Members of the orchestra and audience marveled at his magnificent performance.He is totally blind from birth. I don’t know why a blind person can play piano so good. I think I must study piano more seriously because I cansee.

人工透析患者さんは自業自得ではない。Dialysis patients are not asking for it.
長谷川アナウンサーがブログに人工透析患者は自業自得だから高額な治療費を自分で払うべきだ、それが出来ないなら死んでもらうと書いた。それは暴言であり間違ったコメントである。彼は総ての放送番組から外されたが当然だと思う。彼は透析患者さんはでたらめな生活を送ってきたから腎不全になったのだと書いているがそれは正しくない。病気は食事や運動などのライフスタイルによってのみ起こるのではない。遺伝や、感染、環境、小児期の熱性疾患など様々な要因によって起こる。一つの病気には様々な原因があり、一つのことそれだけが原因ということはない。 彼がなぜそのような考えを持ったのか知らないが、ブログに書く時にはそれが正しいかどうかきちんと調べるべきだと思う。 An announcer Mr. Hasegawa wrote his blog that the patients who are receiving artificial dialysis are asking for it, they should pay money themselves, if they can't , they should die. It is violent language and a wrong statement.He was dismissed from all broadcast programs. It is right, I think.He wrote the dialysis patients have a slovenly life and became a renal failure. It is not correct, disease owes not only their lifestyle food or exercise etc. It owes to inheritance, infection , environment, or childhood fever disease etc. Many causes are present for a disease, there is never one reason. I don't know why he had such thought, but he should investigate it if correct or not when he write his Blog.

ピアノの先生と仲直りしました。I made up with my piano teacher
先週の水曜日、ピアノの先生に、教え方が難しいのでもう少し優しく教えて下さいと言ったら先生は怒って泣いてしまった。以来お互いに気まずくなってしまった。昨日のレッスンの最初に「前回は基本をおろそかにするようなことを言って済みませんでした」と謝った。そしたら先生も自分もしつこ過ぎて済まなかったと謝ってくれた。お互い仲直りすることができて、いつもより楽しくレッスンを受けることができた。相手の人と仲直りするには貴方がまず謝ることだと思う。そうすればうまくいって笑顔を取り戻せることになると思う。I addressed to my piano teacher that her way of teaching is very difficult for me so please teach me more easy, last Wednesday. She angered and cried. We felt awkward each-other from then.I said first at the yesterday lesson " I am sorry that I said as like ignore the elements at the last lesson". She answered me that she also was sorry because she was too persistent. We made up each-other and the lesson went well more than usually. I think to make up with the opponent, you should apologize first. So it will go nice and you will get a smile again.

大隅良典さんノーベル賞おめでとう！！ Congratulation !! Mr. Ohsumi Nobel prize.
The 2016 Nobel prize in medicine or physiology has gone to Yoshinori Ohsumifor his work on autophagy. The biological recycling system breaks down oldor dangerous proteins and turns them into fresh building blocks for thecell. The same process prevents the buildup of clumps of proteins that canlead to devastating neurodegenerative diseases ,like Parkinson disease orAlzheimer's disease.I expected to get the prize Dr. Honjo who discovered the immune check pointPD-1 in cancer cells which stop the immune system of our body. I think it is the nummber1research to cure the cancer in the world. But he was not chosen andMr.Ohsumi was chosen. This is also most splendid happening. He is a quietresearcher handling the yeast.He discovered the autophagy in the yeast on a vacation but a competitionmind. It will be useful for the research of cancer ,Parkinson disease orAlzheimer disease.I was very glad to hear the news. Wonderful!2016年のノーベル医学・生理学賞を日本の大隅良典さんがオートファジーを発見した業績で受賞した。オートファジーというのは細胞の中でのリサイクルシステムで、古くて邪魔になったタンパク質は壊されて新しいものに作り替える現象である。この現象は、神経変性疾患のパーキンソン病やアルツハイマー病（認知症）の原因になるタンパク質を作るのを妨げるプロセスでもある。私は我々の体の免疫システムを働かないようにするチェックポイントPD-1をがん細胞が持っていることを発見した本庶博士がノーベル賞をとることを期待した。それはがん治療におけるずば抜けた世界一の業績だと思う。しかし彼が選ばれずに大隅氏が選ばれた。これもまた最高に素晴らしいことである。彼は酵母のみに取り組んできた地味な研究者である。彼は競争心からではなく、遊び心でオートファジーを発見した。オートファジーはがんやパーキンソン病、認知症の研究に役立って行くと思う。私はこのニュースを聴いてすごくうれしかった。素晴らしい！

免疫チェックポイントを抑制してがんを治す。Cancer is cure to use the Immune checkpoint inhibitor
がん細胞が生まれたら我々の体の中にある免疫細胞だけがそのがん細胞を殺すことが出来る。しかしがん細胞は我々の免疫システムを働かせなくさせるＰＤ－１という物質を作って自分たちを攻撃させないようにする。それで我々はがん細胞を殺すことが出来なくて彼らに打ち負かされて滅びていた。日本人医師 本蔗 佑氏（京大）は我々の免疫システムをゼロにしてしまう物質ＰＤ－１を発見した。PD-1に結合してＰＤ－１の働きを奪ってしまう薬が生産されてがん患者さんに使われている。その薬の名前はオプジーボだ。今はまだすごく高価だが重症がん患者さんが多く救われている。ＰＤ－１の発見者本蔗 佑氏はノーベル賞の有力候補である。今夜それが発表されるが彼がその賞を取ってくれるよう願っている。If the cancer cells are born, our immune cells held in our body only can kill them.But the cancer cell makes a substance named PD-1 to stop the activity of our immune system. So we can’t kill the cancer cells and beaten by them.Japanese doctor Tasuku Honjo (Kyoto University) discovered the substance PD-1 which is able to nullify our immune system. The medicine to combine the PD-1 and to rob the function of the PD-1 is manufactured and used for cancer patients. The name of the medicine is Opdivo. It is still very expensive, but many severe patients are recovered. The discoverer of PD-1, Tasuku Honjo is now candidate of Nobel Prize. I hope him to get the first prize.

歯は抜くべきではない。Don’t pull out your tooth.
高血圧治療中の患者さんが昨日私の外来に来た。義歯を作ってもらってからそれが合わなくてスムーズに噛めず味が全く分からないとのことだった。その患者さんは歯槽膿漏で歯医者さんから抜かなければいけないと言われて抜いて義歯を作ってもらったのである。彼はその歯を抜いたことをとても後悔しているとのことだった。私にも歯槽膿漏があり、噛む時痛かった。ある歯医者さんに行ったらそれらの歯は抜かなければダメだと言われた。別の歯医者さんに行ってみたら抜く必要がなく、古い金冠を外してセメントを詰めてくれたら今は全く痛みはない。私の歯はセカンドオピニオンの歯医者さんによって救われたのだ。ある歯医者さんが歯は抜かなければならないと言っても他の歯医者さんにもう一度診てもらうべきだと思う。A patient who is treated with hypertension came my clinic yesterday.He said he couldn’t notice any taste because he couldn’t bite smoothlyafter making an artificial teeth.He had suffered by pyorrhea , and a dentist pulled out the teeth and made anartificial teeth. He said to me he had regretted to pull out the teeth.I had also pyorrhea and pain when I bite. Some dentist told me my teeth hadto be pulled out. I visited another dentist, he said there is no need topull out and covered a tooth by cement after removed old crown. I have nopain now. My teeth was saved by a second opinion dentist. You should notpull out even a dentist told you had to pull out the teeth. You should go toanother dentist if you were said to pull out your teeth.

精神的緊張から解放された。I was relieved from a mental tension.
私は91歳の重症で死にそうな男性患者さんを受け持っていた。ここ2週間ばかりその患者さんのことが気がかりで心休まる時はなかった。患者さんは40年前から前立腺がんがあり、、最近は重症心不全で苦しめられていた。そこへ持ってきて2週間前に脳梗塞発作に見舞われ半身麻痺になってしまった。左の手足が動かずしゃべることができなかった。家族は人工呼吸や心臓マッサージ、胃ろうなどの延命治療は望まなかった。しかし何日間も夜も昼も心配して見舞っていた。精神的にかなり疲れたと思う。もし患者さんが重症マヒを持ったまま生き残ったとすれば、植物状態になり、人工呼吸や人工栄養を必要になったかもしれない。最近は重症患者さんについて延命治療を希望する家族は少なくなっている。命を長らえても食べたり喋ったり、家族を認識できない患者さんを見るのは辛いものである。昨日その患者さんがなくなった時、その死を悲しんだが、ある意味では患者さんが苦しむのを見ることから解放された。私も精神的な緊張から救われた。I had a 91 year male patient who is severe condition and near to die. He had been attacked by cerebrovascular accident two weeksago and had hemiplegia. He couldn’t move his left arm and leg and couldn’tspeak. His family didn’t want the life -prolonging treatment likeartificial respiration, counter shock, heart massage, artificial nutritionetc.. But they had worried over him day and night long lime. They might betired mentally very much. If he alive with the severe paralysis, he maybecome vegetable human and may need an artificial respiration or nutrition .There are not so many family who want to care their severe patient with thelife prolonging treatment recently. It is miserable to see a patient whocan't eat or speak or recognize one's family after prolonging one's life.When he died yesterday, his family mourned over his death , but they wererelieved from seeing the patient's pain. I am also relieved from a mentaltension.
