Passport Services

The Moline Public Library is an authorized United States Passport Acceptance Facility and accepts passport applications on behalf of the U. S. Department of State.

We offer processing for:

  • First-time applicants
  • Applicants that were under 16 years of age when their most recent passport was issued
  • Applicants whose most recent passport has been expired for more than 5 years
  • Applicants who have lost their most recent passport and cannot submit a renewal form

Those eligible to use the DS-82 Passport Renewal form do not need to go to an acceptance facility to process their application.

We are not a Regional Passport Agency and do not provide physical passports on site.

Passport Applicants

All applicants and relevant parties must be present during the application process.

For minor applicants (under age 16), all parents named on the child's birth certificate and the child must be present. If one parent cannot be present, you must submit a Parent Consent Form (DS-3053), completed with a notarized signature from the non-applying parent/guardian, and a copy of the front and back of the non-applying parent's ID (it should be the same ID that they showed the notary).

Passport Photos

Passport applicants will need a passport photo for their appointment. Passport photos must be a 2” x 2”, color, full-face view photo, taken within the last 6 months. No glasses, hats, headgear, uniforms or camouflage clothing are permitted in the photos.

Passport photos can be brought with to the appointment or be taken at the library at the time of the appointment for applicants 3 years of age and older. Passport photos are printed in pairs. There will be a charge of $15 per pair of passport photos per individual applicant. If an individual needs additional copies of their passport photos, additional sheets (of 2 passport photos) can be printed for an additional $2 apiece.

Hours of Service

Passport acceptance service is offered by appointment only.

Appointment hours:
 Monday through Thursday
 9 a.m. to 7:00 p.m.
 Friday 
 9:00am to 4:00pm
 Saturday
 9:00am to Noon

The last appointment of the day will start 1 hour prior to the end of service.

Applicants arriving 15 minutes or more late for an appointment may be asked to reschedule. Applicants arriving early may still need to wait until their appointed time to begin.

Passport Acceptance Agents reserve the right to stop accepting applications prior to scheduled end time if they judge that processing of the application(s) would continue past closing.

Please allow adequate time for each application when scheduling an appointment.

New Passport Checklist

  • Schedule an appointment - call 309-524-2470 or stop by the Library's Information Desk on the second floor.
  • Complete an Application for U.S. Passport form (DS-11) - applications can be completed online or by hand in black ink
  • Obtain evidence of U.S. citizenship (birth certificate, previous passport, naturalization certificate, certificate of citizenship or Consular Report of Birth Abroad)
  • Make a photocopy of evidence of citizenship
  • Obtain a valid photo ID (current driver’s license, military ID, government employee ID or past passport)
  • Make a photocopy of valid photo ID (for ID cards, a copy of the front and back of the ID on the same side of a single sheet of paper is required)
  • Obtain at least one 2-inch x 2-inch, color, full-face view, passport photo
  • Make sure all applicants and relevant parties will be present at the appointment
  • Have payments ready for appointment - please remember that there are two payments, one that goes with the application that must be a check or money order, and one that goes to the Moline Library
