Boston Scientific stock took a hit today, sliding almost 6% to about US$42 after investors reacted to its Q3 2026 report. At first glance the disappointment centers on adjusted earnings per share of US$0.73 and organic revenue growth of 1.5%, both below what management had guided. For anyone looking beyond the next few quarters, the more powerful story sits in the trailing twelve month picture, where earnings and margins have been running stronger even as the share price has been drifting lower over the past three months.

Boston Scientific Bull Case Meets Mixed Execution
The bullish story argues that Boston Scientific can widen its footprint through higher growth procedure areas and better supply execution.
"Strengthening of urology, endoscopy and CRM through new products, tuck-in deals such as Valencia and Nalu, and improving supply chain execution increases the contribution from these segments and can smooth revenue, support operating margin expansion and support the company's double-digit EPS growth targets."
Latest numbers only partly line up with that ambition. Neuromodulation, helped by Nalu, grew 13% and IO&E delivered 5% growth, which fits the thesis of higher value procedures gaining traction. Yet urology declined 3%, endoscopy slipped 1%, and CRM fell 2%, while adjusted operating margin contracted 150 bps. That mix suggests Boston Scientific is progressing in some targeted niches but has not yet turned the broader segment and margin story into consistent execution.

Boston Scientific Bear Case Finds Real Execution Gaps
The cautious view argues that WATCHMAN and electrophysiology at Boston Scientific could turn into chronic drags rather than temporary soft spots.
"Reimbursement and hospital capacity constraints, especially for stand alone WATCHMAN procedures and sacral neuromodulation, may limit how much of that demand translates into higher net margins."
Q3 results lean toward that worry. WATCHMAN revenue fell 6% with stand alone volumes down by the high teens, and EP declined 5% with U.S. down 8%. Adjusted operating margin compressed 150 bps. Management now flags continued WATCHMAN and EP weakness into 2027, which means this risk is no longer just theoretical in the numbers.
Hunt For Quiet Mispricings
