iRhythm Holdings, Inc. IRTC is pairing sustained revenue growth with better operating leverage, giving investors a stronger fundamental setup than a year ago. Second-quarter 2026 results showed healthy demand, widening margins and positive free cash flow.

IRTC's Q2 Growth Strengthens the Bull Case
IRTC generated second-quarter revenues of $224.2 million, up 20.1% year over year. Growth was driven primarily by sustained volume demand across the customer base, with newer channels adding to the expansion. Revenues also topped the Zacks Consensus Estimate by 2.2%.
The result extended iRhythm's run of more than 20% revenue growth to seven consecutive quarters. Management raised full-year 2026 revenue guidance to $880-$890 million from $875-$885 million, with volume expected to remain the main growth driver.

iRhythm's Margin Gains Improve the Earnings Setup
Profitability improved alongside the top line. Gross margin expanded 160 basis points year over year to 72.8%, helped by operational efficiencies, product mix and scale benefits from higher volumes. Adjusted operating expenses were essentially flat at $145 million.
Adjusted EBITDA reached $43.3 million, or 19.3% of revenues, improving by more than 1,000 basis points year over year. Free cash flow was a record $37.5 million. Scale, workflow optimization, manufacturing automation and disciplined spending are increasingly translating revenue growth into operating leverage.
IRTC's Expansion Strategy Broadens the Opportunity
iRhythm is pushing ambulatory cardiac monitoring beyond traditional cardiology into primary care, value-based care and population health. Innovative channels were its fastest-growing channel in the second quarter, with new partners continuing to join across these settings.
Workflow integration is supporting that expansion. About 60% of second-quarter volume came from electronic health record-integrated accounts, up from roughly 53% in the first quarter, while nearly 80 of the company's top 100 customers were integrated. These connections can reduce ordering friction and support broader adoption.
iRhythm Still Faces Reimbursement and Execution Risks
Reimbursement remains a key uncertainty. In Japan, better reimbursement took effect June 1, but international expansion still requires clinical evidence and market-specific reimbursement progress. Across Europe, reimbursement remains fragmented, while U.S. Medicare and commercial payers continue to scrutinize utilization and cost.
Execution risk is also rising as newer channels scale and iRhythm prepares to integrate Vital Connect. Competition remains active. Koninklijke PhilipsPHG offers ambulatory cardiac monitoring, including mobile cardiac telemetry for up to 30 days. Boston Scientific CorporationBSX also markets BodyGuardian remote cardiac monitors across Holter, event and mobile cardiac telemetry applications.
The planned Vital Connect acquisition adds integration demands while Zio MCT clearance is targeted for the first half of 2027. iRhythm also remains subject to an FDA warning letter, with the timing of any agency action outside the company's control.

IRTC's Mixed Style Scores Support a Measured View
The bottom line is that iRhythm's growth and profitability trends have improved, but the risk-reward picture is not clean enough to support an unqualified bullish stance. Reimbursement, product execution, acquisition integration and regulatory follow-through still matter.
