Pfizer( NYSE:PFE ) shares have risen 24.41% year-to-date to $29.68, and the stock still yields 5.82%. That yield leaves retirement-income investors with one big question. Pfizer is paying out more than it brings in as free cash flow.

Dividends Outran Free Cash Flow in 2025

In 2025, Pfizer paid $9.8 billion in dividends. Its free cash flow (operating cash flow minus capital spending) was $9.1 billion. Dividends ate up 107.7% of free cash flow. The market ratios show the same thing: the dividend yield is higher than the 5.40% free cash flow yield.

On earnings, the gap is wider. Trailing diluted EPS is $0.76, while the annual dividend is $1.72 per share. The quarterly payout is $0.43, up from $0.42 in 2024.

Why Reported Results Trail the Payout

One-time charges explain much of the gap. Second-quarter 2026 GAAP results showed a net loss of $248 million. That loss came after a $3.8 billion impairment tied to a failed Phase 3 trial for sigvotatug vedotin and $867 million in legal charges. An impairment is a write-down. It brings down reported profit, but no cash leaves the company.

Timing also played a part. Operating cash flow for the quarter was $835 million, while dividends paid came to $2.451 billion. In the same quarter, Pfizer made its final TCJA repatriation tax payment of about $2.6 billion. Adjusted EPS of $0.77 beat the $0.68 estimate.

Bourla Says the Dividend Will Survive Every Scenario

On the second-quarter call, Chief Executive Albert Bourla replied directly to concerns about a dividend cut:

"We feel extremely confident that, even in the most stretched scenarios that we are running, we will be able to maintain our dividend. I want once and for all to make that clear to all: the dividend will be maintained and, after the LOE period, will start growing again."

Cecile Guegan, who was named new interim CFO, described the priorities this way:

"Reinvesting in the business at appropriate returns, maintaining and, over time, growing our dividend and preserving optionality for future value-enhancing actions, including share repurchases."

The numbers support that pledge. Pfizer raised its 2026 revenue guidance to $60.5-$62.5 billion. Its adjusted EPS guidance of $2.80 to $3.00 covers the dividend. The company bought back no stock in 2025 and plans none in 2026. Savings from its cost restructuring program now total $6.7 billion through 2029.

Debt and Fading COVID Sales Are the Pressure Points

Its financial position is less comfortable. Pfizer held $976 million in cash and $10.723 billion in short-term investments at June 30. Total debt was $63.194 billion, and net debt was 2.76 times EBITDA. Management expects leverage to stay "around current level or modestly higher" as major drugs lose patent protection. It also brings down its 2026 COVID revenue outlook to about $4 billion, down from $5 billion.

Peers face the same pressure. At Bristol Myers Squibb( NYSE:BMY ), Revlimid sales fell 47% in the third quarter. Even so, Bristol raised its $0.63 quarterly dividend for a 17th consecutive year and produced $4.2 billion of operating cash flow in the quarter. Merck( NYSE:MRK ) grew third-quarter revenue 4.68% to $18.085 billion, though sales of Keytruda, its biggest drug, slid 5%.

Verdict: Secure for Now, With No Growth Until 2029

The dividend looks safe in the near term. Adjusted earnings cover it, one-time tax and impairment costs explain most of the shortfall, and buybacks remain on hold. Meanwhile, the balance sheet is filling part of the gap. Bourla expects Pfizer to "return to growth from 2029 onward". Until then, investors should expect little or no dividend growth. The number to watch is full-year 2026 free cash flow, compared with a dividend bill of about $9.8 billion.
