IONS Secures FDA Nod for Rare Brain Disorder Drug Zanvastro

Ionis PharmaceuticalsIONS announced that the FDA has approved its RNA-targeted therapy zilganersen to treat both pediatric and adult patients with a rare neurological condition called Alexander disease (AxD). The drug, which will be marketed under the brand name Zanvastro, is expected to become available in the United States in the coming weeks.

The FDA's decision comes ahead of its previously assigned target action date of Sept. 22, 2026. It is supported by positive results from a phase III study, which showed that treatment with the drug achieved a statistically significant and clinically meaningful stabilization in gait speed.

The approval is particularly significant because Zanvastro is the first disease-modifying treatment for AxD, a condition where treatment has historically been focused primarily on managing symptoms. It also marks Ionis' first independent launch in the neurology space.

AxD is a rare disorder that affects a type of cell in the brain called astrocytes, which play multiple roles in supporting neurons and oligodendrocytes. People living with this condition experience cognitive dysfunction and progressive neurologic deterioration, including difficulty swallowing and the inability to control muscles for large movements.

Zanvastro is now Ionis' third wholly owned marketed drug, alongside Tryngolza (for severe hypertriglyceridemia and familial chylomicronemia syndrome) and Dawnzera (for hereditary angioedema), both of which gained approval over the last two years. These commercial launches are part of Ionis' broad strategy to deliver a steady flow of wholly owned medicines to patients and reduce reliance on collaboration partners for revenues.

Priority Review Voucher Adds to the Value

Alongside the approval, the FDA awarded Ionis a rare pediatric disease priority review Voucher (PRV). The voucher can potentially be used to accelerate the FDA review of a future marketing application or sold to another company, providing Ionis with an additional source of potential value.

The PRV is particularly noteworthy for investors because its economic value is separate from Zanvastro's commercial opportunity. The voucher could therefore provide Ionis with additional strategic flexibility as it advances its broader pipeline.

Limited Patient Population Caps Ionis' Commercial Opportunity

Despite Zanvastro's clinical importance, its commercial sales potential is constrained by the rarity of the indication. Ionis itself acknowledges that AxD is an ultra-rare condition that affects approximately one in one to three million people worldwide. As a result, we do not expect Zanvastro to generate significant commercial sales when compared with Ionis' other wholly owned products.

Ionis also has limited direct commercial exposure outside the United States because it has out-licensed exclusive ex-U.S. rights of Zanvastro to Italy-based Recordati. Under the agreement, Recordati is responsible for commercialization outside the United States, while Ionis retains U.S. commercial rights.

Over the past 30 days, loss per share estimates for Iovance Biotherapeutics have narrowed from 60 cents to 49 cents for 2026. Over the same period, estimates for 2027 loss per share have narrowed from 36 cents to 29 cents. IOVA shares have skyrocketed 219% year to date.

Iovance's earnings beat estimates in three of the trailing four quarters while meeting them on one occasion, delivering an average surprise of 16.83%.
